Medical Requests for Under 16s

Under 16s can now use Smart Triage to submit new medical and administrative queries. Unlike adults, Under 16s will not receive an immediate appointment at the end of the triage questionnaire. The answers you submit will be sent to the duty clinician to review and offer an appropriate appointment.

Medical requests are reviewed on the day based on urgency. Administrative requests may not receive an immediate response, and will be dealt with in date order. Responses are generally sent via email or text message, so please let us know in the request if you would like to be contacted in a different way.

If you are submitting on behalf of someone else, please enter their details so that we can match it with the patient record.

How to use Smart Triage for patients under 16

There is one new option on the main page: Child request. Click ‘Select’ to submit a request for a patient under the age of 16.

There are eight options to choose from – please choose the most appropriate for the query:

  • New medical problem
  • Medical problem that is not new
  • Follow-up for a medical problem
  • Repeat prescription
  • Medicine review
  • Forms and paperwork
  • Immunisations (vaccines)
  • Other request
