Non-urgent advice: How to book
1) Call 01909 500 233.
2) Press option 2 to access the automated system
3) Press option 1 to book an appointment
4) Enter your date of birth and registered phone number
5) Press option 1
6) You will be asked which site you wish to book, choose one suitable for you.
7) You will be asked if you have a PIN. Press 1 (yes).
8) Enter the PIN on the SMS/letter
9) Listen to available appointments and book. Press 3 to skip to the next day.
What is a medication review?
A medication review is an in-depth look at your medication by a qualified pharmacist working within the practice. They will review any symptoms you may have and assess recent blood tests/monitoring. Their goal is to make sure that the medication your are taking is still suitable for you, and being prescribed safely.
What do I need to do before booking my review?
Blood Pressure
If you have not had a blood pressure check within the last three months, please attend your nearest surgery to use the blood pressure self-service pods (available at Carlton, Larwood and Westwood). When taking your blood pressure reading, please take a seat for five minutes which will allow your blood pressure to normalise, and remove any coats or thick jumpers.
Long Term Condition Reviews
If you have recently received an invitation for a Long Term Condition review, you should book and attend this before booking your medication review. The following conditions are eligible for condition reviews:
- Diabetes
- Asthma
- Chronic Obstructive Pulmonary Disease (COPD)
- Hypertension
- Atrial Fibrilation
- Severe Mental Illness
- Learning Disabilities
- Dementia
- Chronic Kidney Disease (CKD)
- Coronary Heart Disease (CHD)
- Stroke and Transient Ischemic Attach (TIA)
- Heart Failure
- At-risk of Diabetes monitoring
- Thyroid monitoring
Why are reviews important?
As a prescriber, we are responsible for ensuring that medication is safe and appropriate to prescribe to patients. Monitoring helps us ensure that each medication is not having any side effects or negative impacts on your health. We may decide to reduce the issue quantity, pause or stop prescribing for patients who do not attend the necessary tests, as we are unable to confirm it is being used safely. m
