Your invitation to a FREE NHS Health Check

Dear Patient

We are writing to inviting you to attend for your FREE NHS health check.

The check is to continue to assess your risk of developing diabetes and to assess your 10-year risk of heart disease.

If there are any concerns identified, we can discuss this and take appropriate action.

By taking early action and following any advice you have been given, you can improve your health and prevent the onset of these conditions. There is good evidence to support this.

How do our NHS Health Checks work?

We have recently improved our process so that more patients can benefit from having an NHS Health Check

  1. Received NHS Health Check Invitation letter
  2. Attend the surgery BP Pod to measure your weight, pulse and blood pressure
  3. Complete a short health questionnaire
  4. Attend your nearest hospital for a blood sample (Doncaster, Bassetlaw or Montagu)
  5. Receive your results in the NHS App.

What does an NHS Health Check look for?

The NHS Health Check is designed to identify early signs of common but preventable health conditions. It assesses your overall health and helps to spot any risks of developing the following:

  1. Heart Disease
    By evaluating factors such as cholesterol levels, blood pressure, and lifestyle habits, the check can identify your risk of cardiovascular disease and suggest ways to keep your heart healthy.
  2. Diabetes
    Through blood tests and lifestyle assessments, the check can spot early signs of type 2 diabetes or prediabetes, enabling you to make changes to prevent or manage the condition.
  3. Dementia Risk (for patients aged 65 and over)
    The Health Check includes advice on reducing your risk of developing dementia by focusing on lifestyle improvements that benefit brain health.

By understanding your health risks and taking steps to address them early, you can significantly reduce your chances of developing these conditions. Regular Health Checks are a valuable part of maintaining long-term wellbeing.

How the Results Help You

After completing the Health Check, depending on the results you may receive personalised advice based on your results. This might include guidance on:

  • Improving your diet and physical activity levels
  • Managing stress and sleep
  • Addressing alcohol consumption or smoking habits
