Registering at a GP surgery is a straightforward process. You’ll just need to fill in two forms (or just one if registering online), and provide some identity documents. We’ll then put your details onto our system and you’ll be able to access the services at our outstanding rated surgery.
Who can register?
We are currently accepting new patient registrations from anyone currently residing in our practice boundary. Our boundary includes most of Worksop and the surrounding villages.
We accept registrations from people with no fixed abode who are in the Worksop area.
How can I register?
You can register online or in-person at the surgery. Registering online is the easiest and quickest way to send your details.
We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery. Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper Registrations
If you would like to submit a paper registration, we need the PRF1 form completing, which you can download below. We also need some proof of identity, such as a passport, driving licence or a birth certificate.
Once complete, you can drop them into your nearest surgery in our group. We do not recommend posting them, unless by recorded delivery.
What next
Please ensure you’ve provided an up-to-date contact number so we can keep you updated. It can take up to 7 working days to register, but if you need urgent healthcare please contact the surgery or submit a request using Smart Triage. Private and Temporary Patients
Private Patients
Private Patients can be seen at Larwood Health Partnership.
There will be a charge for consultations as a private patient. If you require any tests or follow on consultations further charges will apply. Please contact our reception team using Smart Triage or call us on 01909 500 233.
Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You cannot register as a temporary patient at a practice in the town or area where you are already registered.
